Even the sun came out to greet us! We went straight to Saco do Céu, Paul's most favorite spot on Ilha Grande. And as soon as we got there, as if by magic, the few boats that were around, left. The sun came out and I did what I thought was going to be easy, but was one of the hardest things I ever had to do! Although I know that Paul's essence was no longer there, and I can feel him around once in awhile, opening the box and jar and actually seeing his remains was incredibly hard. Once they started to be spread, Paul's ashes just melted into the sea, becoming one. So fitting to have his "final resting place" be in such a spectacular and meaningful spot. Thank you to each and everyone who was on the boat and to those who were with us in spirit! It was a wonderful day and so much like Paul. Even the dolphins made an apearance in the distance! I want to take the opportunity to thank everyone who has been there for me and Paul all these years, and specially these last 6-7 months. Thanks doesn't cover the depth of the feeling. I said my final "physical" good-bye to Paul today, but he will always be with me, in my heart. And this will also be the last time I post anything on this blog. This is good-bye, for now!

Dear all,

Paul's ashes will be spread at sea in the Angra Bay on Wednesday - the 3rd of March. I have rented a saveiro (big boat), which will be ours the entire day. It will leave the Angra docks in the morning (probably between 9:00 and 9:30 - still to confirm). We will be heading towards Saco-do-Céu where Paul wanted his ashes spread. We will have ice, drinks and fruits on board. On the way back, we will stop for luch at a restaurant nearby (each will be responsible for their own), and depending on the weather we will cruise around the bay a bit, before heading back, before dark. The boat will fit up to 80 people, but I know some want to follow in their own vessel. If you need to coordinate meeting times, please let me know. For those who want to go on the saveiro, I will need to know the full names of each person, as soon as possible, through this email: rampjd@terra.com.br. I will arrive in Angra on the 2nd in the afternoon and will be staying at a local hotel, probably 2 nights. If you need housing there as well, let me know, and I can help you find something suitable. I know it is going to be difficult for some to be there due to it being in the middle of the week, but I hope many of you can make it! I will be waiting on your answer. And please, forward this email to anyone that you know who knew Paul, as his computer burned and I lost all of his email contacts! That 2010 be a great year to all. Those would have been Paul's wishes as well! And in light of his wishes, finally a date for the spreading of Paul's ashes has been set. I really like patterns, so my initial thought was to have the ceremony on the 3rd of March in Angra. Paul was born on 08/08, died on 10/10 and so 03/03 seemed a good day to do the ceremony. That seems to work for most of the close family, so 03rd of March it is. Paul loved Angra and Ilha Grande, and especially a spot called Saco do Céu. I have no idea if, with all the tragedies that are happening there at the moment, this will even be accessible and I have not arranged anything in terms of boats yet, but I would appreciate any help/input/offer from all of you. I would like to start setting things up, so that we have a better plan for all those who would like to be part of this. Let me know what you think and how we could get this last of Paul's wishes filled. Lots of love,

Renee

Renee

Thank you to all that have come to be with me on Saturday and Sunday, last week, in Guapimirim and, at the church service, in Teresopolis. At the last minute I decided, and managed, to get to England for Paul's funeral service over there, which was really special, just how Paul would have liked it! There are so many who made this possible and my deepest thanks go to all of you, including Father Mark, who did not know Paul, but did a fantastic job in celebrating Paul's life and his connection to God. Surprisingly, Paul's ashes have been released already and, hoping that everything will go smoothly, I will be returning his remains with me tomorrow, when I return to Brasil. I will start arranging for the ceremony of spreading the ashes somewhere in Angra, but have no time set for this. I will keep you posted.

Lhes manterei informados.

This Sunday - October 18th, there will be a service at 10:00 in Teresópolis, in the Igreja N.S. de Fátima, Rua Tietê - Bairro de Fátima, in front of the Casa de Cultura. He died in his sleep. He was doing fine yesterday, having been cleared by the physician to travel home on Monday. He went to bed last night and around midnight his sister checked on him, and he was ok. This morning, he was found in a very peaceful way, looking as if he was still asleep and well rested. No signs of struggle or trouble. He litterally went to sleep eternally. This was also the first night in months that I slept really well and deeply. I am in Rio, assisting in a SomatoEmotional Release course and slept in a hotel, by myself. I dreamt the whole night about Paul and we were swimming with the dolphins. It was the first time in months that my mobile was not against my body and it was on vibration mode, so I did not hear the first calls. My mother, who is in Brasil at the moment, was notified and managed to find the hotel number where I was and called the room. Although the news came as a shock at 5am, it did not come as a surprise: he had been with me all night long and we had prepared ourselved all along that this might indeed happen. Yesterday he and his daughter managed to have a long and frank talk and "clear the air", which is such a blessing! He managed to see the people he wanted to see the most, and say to them what needed to be said. What a gift that is!!!! How often do we not get the chance to do that? Or we get the chance and don´t take it!!! I am hurting, of course!!!! But I could also not be at a better place then where I am right now! I HAVE OPTED TO STAY WITH THIS WONDERFUL GROUP OF PEOPLE here in Rio, doing this course. These are people that knew and loved Paul, and that are grieving just like all of us are, and who are, above all, being the love and support to me in a way that is beyond words! I am not going to England. And I am not going home (Guapimirim) until Monday afternoon! Paul had requested to be cremated and that will be done in England, with his family, his older children and grandson. Somehow we will get his remains to Brasil, in due time, and we will be arranging for services on this end. He had asked to have his ashes spread in the sea, so I will get that organized eventually. I will continue to post things on this blog as they come up. I am reachable on my mobile, via this blog and via my own email dra.renee@terra.com.br. Please understand, if I do not answer either of them! And again, I am so grateful I have had the chance to love this wonderful man, be a part of his life and to have had the opportunity to build something so beautiful alongside him! I am also grateful for the peacefull passage and the chance he had to be with the ones he needed to be with in the end!

Ele morreu no sono.

Not much is happening at the moment, in terms of health, but Paul is managing to spend good and valuable time with his children and family. The last two days he has not been too comfortable, due to intestinal troubles, but I hope those will be solved during the night. As usual, Paul took his time to tell his sister of his discomfort, that is why it took so long to address it!

I just received a few photos of Paul in England and I am sharing them with you. He has gone to see his children, grandchild, his family and friends who are overseas, and to be pampered and "fattened" up. I spoke to Paul twice today and he sounds much better, having had the chance to rest a bit after the trip. He did have to go to a physician today, because he opted to have oxygen at night, which makes for an easier night of rest and sleep. The oxygen was approved and delivered to his sister's house, so he is all setup. And he will need the clearance of another physician on his way back, mid-october. That should be it for medical appointments! For the rest, he should enjoy his time over there to the fullest!

I have just spoken to Paul. This lead to me finally being able to access some fast information about esophageal cancer and it's care. I am pleased to announce, to the surprise and relief of many, that, although we live "at the end of the world", the treatment options, medications and protocol setup for Paul are exactly the same as being used at the Mayo Clinic in the States and other major cancer treatment institutions around the world! Thanks to all of you who have been sending me updated info on the latest interventions. Please continue to do so, as our oncologist is very open to other ideas! The other news today is about Paul: he has been cleared to travel to England and he will be leaving tonight. He is travelling alone, as per his own request, much to my chagrain. However, he will have all the medical assitance required at British Airways, which has been organized in advance. My only concern at the moment is the flight. I know he will be well taken care of and pampered, once in England. He will stay with his sister's family in the South of England. I will update the blog, as things come up. Thank you again for all the prayers and positive energy! It has made it possible for us to "survive" so far!!!! Lots of love to all!

Minha única preocupação no momento é o voo.

Paul did not have a tranquil night, but in the end he managed to caugh open part of the left lung again. He does not really agree with Paul's trip, but Paul is stuck on the idea. He recommended a second bronchoscopy to be made, for a few reasons: attempt to open the bronchus again; see if the closure is being caused by the tumor or some other reason; evaluate the need for a bronquial stent to maintain it open. He called the doctor who does the bronchoscopy and he was of the opinion that it could indeed be done, but definitely not two/three days before a flight! Better to do it after Paul returns and before the chemo/radiation starts. As Paul is so vehement about going to England and he is doing well, even with the left lung almost closed, the doctor reluctantly made the following agreement: we will see him again on monday morning and he will then give his definite word on the trip, so Paul can be on the evening flight straight to London (or not). The ticket is booked and paid for (thank you Denis & Kay!!!!) and I spent the rest of the day taking care of all the details: wheelchair assistance, special diet, oxygen on board, etc! Never knew it took so much paperwork, hasle and calls to arrange for all of that. Anyway, that is the news for the day. I hope by monday I can report to you that Paul is safely on the flight and, better yet, that he is in London on Tuesday, without any major events!

We just got back from the various doctors. The results are a bit ambiguous, you will understand in a second. First we went to the hospital for a review of Paul's lung-status and x-ray, and to pick up the results of the other tomographies which were looking for spreading. The tomographies are clear! Nothing in any of the other organs, just really the site around the esophagus and the left bronchus. Paul went for a chest x-ray. Our pneumologist was not available, but the x-ray showed that the left lung has partially closed again. Of course, we do not know if this is due to yet another piece of dislodged material or if due to the tumor that might be compressing the bronchus. We would only know for sure via a second bronchoscopy or if Paul caughs the lung back open again. In any case, Paul is doing fine, not short of breath, but caughing a little - not a lot! As our pneumologist was not there to take any other actions, we were allowed to leave and went straight from the hospital to the oncologist, for our first official consultation. Paul is at 78.3kg weight. A total loss of 12kg in the past months. But he has already gained two since the last hospitalization! She had a long phone conversation with the surgeon who placed the stent, right in front of us, and they agreed that there can no longer be any leakage from the esophagus to the lung through the opening that was closed with the stent. And chemo/radiation could be started in about 15-20 days (apparently they normally would wait about 4 months after the placement of the stent, so that the patient can get stronger before the treatment). So she has cleared Paul to go to England, leaving Monday and returning on the 12th of October. He will than undergo his radiation evaluation on the 15th. If approved, chemo and radiation will start on the same day, either the 19th or the 26th. He will undergo 5 weeks of treatment, radiation being 5 days per week (monday to friday) and chemo for 4 days in the first week and 4 days in the 5th week. If the radiation is ruled out, he will just have the chemo, still in the same format. And then he will be re-evaluated. We are still not sure if the insurance will cover all this, and we are also still not sure if Paul will really be allowed to go to England, as his lung did not look too good, despite him feeling well. We have arranged to see our pneumologist tomorrow morning, to have a clearer idea of what to do about the lung, either now or during Paul's trip. I will let you know tomorrow! Now I am going to sleep! Good night.

Agora vou dormir! Boa noite.

Things are going well on our side. We are back "online" but are now without phones, since a huge storm last night! The good thing is that Paul had the best night in months last night, sleeping soundly without any pain, and got up with a lot more energy. We ventured up to Teresópolis, to have some lunch and get some meds. We were supposed to see the pneumologist today, but that got postponed to thursday, because we will see him together with the endoscopist, to make sure that Paul's stint is definitely closed and not allowing liquids to sneak to his lung. They will also evaluate him for the placement of a specific feeding tube, which he will need once he starts the chemo and/or radiation. At the same time, we will be receiving the tomographies of his belly and pelvis done on Friday in the hospital, which, as far as we have been informed, are clear. If all is well (and it will be!) Paul wants to leave for England towards the end of the week, staying there for approximately 15 days. A lot of people have been contacting us with different alopathic and alternative treatment options. We are very interested in ALL our options, so please, if you have info, send it to us in an email with as much detail as possible (where, who, what, when and how much). Paul is already under the care of a Homeopath and is starting on those as soon as they are ready. They will help build his strength and will not interfere with the chemo and/or radiation. More news later.

We are "borrowing" internet at friends, therefore our notes are quick and short. The oncology visit was to have happened on tuesday, then it was to happen yesterday before noon and still no one showed. I ended up going to the department and talking to one of the doctors, who wasn't rude, but was not at all helpfull. It did not bother me as much, because we had already started to arrange for another oncologist, from outside the hospital, to come see Paul. But the staff of the floor where Paul is staying got so upset, that the director of oncology was called and all of a sudden two visits ended up happening today! But they weren't willing to answer very specific and objective questions, which will actually decide what the course of action will be on Paul's side. They confirmed radiation nor surgery are options, but want him to try chemotherapy, even though it is known that this kind of tumor does not respond well to it. The trial would be for three months and then they could tell us if it will or not work. They did not want to go into specifics of size, spreading, speed of growth, nor what would happen if Paul opted NOT to do the chemo. So it was decided that Paul was going to have a tomography, to make sure there is no other lesions beside the esophagus, and for that he needed to be without food for 8 hours. As he had just had lunch, the exam was postponed to tomorrow morning. And we have arranged for the oncologist from outside the hospital to come by tomorrow morning, probably before the exam. We will see what she has to say. As soon as there is news, I will post it on the blog.

MORE NEWS

What a day! Pneumologist came and confirmed there was sound of air in the lung. x-ray was ordered and indeed, three-quarters of his lung was back and functioning! The bronchoscopy which had been ordered to open the lung, was cancelled. Paul was placed on an intravenous drip with antibiotics, just to make sure no infection would take its chance. Although Paul has no signs of infection, no fever and no alteration in his blood pressure, the fact that the left lung has stopped working is cause for concern. And the lack of air is leaving him exhausted. The doctor wants to know what is causing this and wants to try to re-open the lung. So far, the good news is, that it will be a short stop (probably only 2 days), and, with Paul being in the hospital, we have priority in the Oncology consultation. The main oncologist will probably come to see Paul today. Paul has spent a tranquil night, with an oxygen mask and very little coughing. I'll post more news later.

Update on Paul

Dear all, it has been a few days without updates, because there was nothing to report. Paul is home, trying to get some weight and rest. Yesterday he started coughing again and today we ended up at the hospital once more, because the coughing makes him really weak and sore. The x-ray shows his left lung has stopped. We are not sure why, but are on our way to the pneumologist at the end of the day. The oncology team has contacted us today and they have scheduled Paul for the 24th of September, but we are trying for an appointment still this week. I will let you know how it goes.

PAUL IS HOME! PAUL ESTÁ EM CASA!

Sorry for not updating the blog earlier, but it has been a bit hectic. Paul has been allowed to go home yesterday. He was taken off the venous port and is taking all meds by mouth. We are setting up the consultation with the Oncologist for early next week, as they need to study his case before seeing him. The results of the biopsy finally arrived and the growth is malignant, originated from the outer-layer of the esophagus, moderately agressive and it does not respond well to chemotherapy. We will not know what other options will be available, until we meet with the oncologist. At this time Paul is resting at home, trying to get stronger and gain a bit of weight. As he gets tired quickly, we are avoiding visitors at this time, and he is still not speaking too much, so he will not be picking up the phone. As things progress, I will be updating the blog. Thanks for all the support and love that have kept us (ME) going these past few weeks! My special thanks to the VM class this weekend!!!!! Lots of love!

The stint is nicely in place and covered the hole to the left lung, immediately allowing him to swallow again. Today they will start Paul on regular foods, slowly getting his stomach used to food again, and they will monitor how he reacts. When the stint was placed, opening the esophagus, the doctors were able to go through with the endoscope and discovered that the lesion on the esophagus reaches all the way to the beginning of the stomach, but the stomach is intact! This lesion is coming from the growth in Paul's chest, which we have no identification for yet. The biopsy results have not returned, because, instead of just identifying the tissue removed on the 26th of August, they have gone for the full scope of testing: what it is (good or bad), where it comes from, speed of growth, agressiviness and which therapeutic intervention it reacts best to. This entire process takes 15 days. Which means by Tuesday or Wednesday we should have an answer. The doctors who operated on him yesterday are confident that Paul will be able to go home either tomorrwo or, by the latest, Tuesday. By then we will already be in the care of an Oncology team, to see what can be done about this growth in Paul's chest. Thank you for all your prayers, all your support, all your love. It is what has helped me to stay strong, and what has helped Paul get through this in such a speedy and awesome way! Thanks is just not enough!.......

05/SET 3pm

Paul is out of surgery, in his room, awake and without his feeing tube. This caused him to be without feeding for the entire day, but at least the nutricionist found a better product, with more calories and easier absorption. Paul lost close to 10kg and need to be strenghtened up. Paul had a restful night, which was great and this morning the big news came in: the surgeon would be available to do the implant of the stint on Friday - TOMORROW!!!!! So the whole day was spent on doing pre-op tests. The procedure is not really going to be an operation, because it will be done via endoscopy, but they will have to put Paul under general anesthesia, as it will be very painful. The insurance is not going to cover for the stint, nor for the surgeon's services, but that is not going to stop us! Everything was going as planned, until 7pm tonight, when the supplier of the stint announced they did not have the piece available and that it would take 15 days to get here!!!!! Both the surgeon and the pneumologist in charge of Paul's case went crazy looking for another option. They just called, telling us they managed to find the material at a different supplier, 30% cheaper (YEAH!!!!), but they can only have it delivered at the hospital by tomorrow afternoon. So the surgeon has decided to do the surgery on Saturday. I do need to add that this weekend is a long weekend in Brazil, as monday is a national holiday. That the guy is willing to give up his weekend is more then fantastic - it is a blessing!!!! Thank you so much for all the support, all the emails, all the prayers!!! Despite all this mess, there are so many awesome things happening and we can only be humbly thankful!!!! Tomorrow will be a resting day for all of us and I will let you know on saturday, as soon as there is news.

O cirurgião e o pneumologista encarregado do caso do Paul ficaram pirados, procurando outra opção!

WEDNESDAY / QUARTA - 02/SET

WED-02/SEP: Thank you so much for all the prayers and positive thoughts!!!! We found not only the stint, but also the doctor to do it, and best of all, in Teresópolis, by next week!!!!! That is the great news. The not so good news, is that Paul will not be allowed to leave the hospital today, as expected. His status is not yet stabilized enough. But, the good side of it is that, if he stays until the implant, the insurance will cover most of the surgical procedure, except the stint itself and the doctor's trip and fee. That does help immensely!!!!! We have no idea yet how much this will cost, but just not having to pay for the hospital stay, the surgery room, the anesthethics, etc, is already great. More news later!

TUESDAY: The day did not start out well - Paul's veins are and none of them could be accessed to minister the antibiotics and the other meds. By this afternoon, they had to do a little procedure and put in a deep-vein port. The breathing has gotten a bit harder again, probably because of the lack of antibiotics since last night, and another x-ray was just ordered. No news on the biopsy yet. They have given us until thursday. Also, we have found two professionals who could do the esophageal implant, but we now need to see about funds, as we are not sure how much the insurance will cover and where they would like to do the procedure. Apparently they have the material. Paul is handling things great, although he was very upset about the vein access not working. He has been online, off and on, on MSN rampjd@hotmail.com.

Paul - Monday/Segunda

Monday - 31/Aug: Paul is perking up since the feeding tube has been working. What a difference food makes! Things were going well yesterday, until he had an allergic reaction to something in his drip. The bad news is that it immediately caused him to have an asthma attack and become short of breath again. The good news is that the drip has been suspended all together, which is a great relief, since he has been on it since last week Monday. Today he is doing a better, but the emotional aspect is starting to take it's toll. We have yet to hear about the biopsy, which might be today. His respiratory status is improving on a daily basis, and we are hoping he will be allowed to go home on Wednesday.

Paul - Sunday/Domingo 30/Aug

Sunday - 30/Aug: Sorry that I did not send any news yesterday, but it was too hectic a day! Now the news: after going really well on friday, due to the bronchoscopy, that evening a feeding tube was placed into Paul's stomach via his nose. All in an attempt to stop food and drinks getting down into his left lung. He is not allowed to have anything via his mouth, including his own saliva! Yesterday was spent making sure the tube was in the right place, that his lung was indeed getting better with the stop of any drainage seeping from his esophagus. We are hoping, that with no passage, the opening between the esophagus and the lung will have an option to start closing. As soon as Paul's respiratory status has stabilized (he is still very weak and not receiving visitors), we will start looking into a more permanent solution for this leak. The doctors are looking to find a specialist who would be able to place a tube (stint) into his esophagus, which would mantain the esophagus open against the pressure of the obstruction and close the connecting hole with the lung. Once we have that in place, Paul will be allowed to eat again. Apparently there are only 3 professionals that do this in Rio, and the stint itself can be hard to find. So we are asking for your specific thoughts in that regard. Paul is taking it all very bravely and is aware we have a long road of recovery ahead. Lots of love and again, thanks for all your support.

Blog: www.pauldelaney.blogspot.com

Friday / Sexta - 28/Aug

Friday 28/Aug: Paul underwent a bronchoscopy today, which was really positive in the sense that they were able to remove most of the blockage of the lung. Immediately he was able to breath better and consequently feel better as well. He is no longer on the oxygen tube, but just on a general breathing mask, to maintain his airways moist. The exam showed an opening between the esophagus and the bronchial passage, which has allowed food and drinks to seep into the lung, causing the pneumonias. At this moment we are looking into ways of preventing this to happen, until more definite measures can be taken. Paul is doing excellent since the opening of the lung, despite being very tired. He will continue in the hospital to at least finalize the anti-biotics (should be by wednesday). No news on the biopsy yet.

Thursday/Quinta 27/Aug

Thursday 27th: Paul slept very well last night and his respiratory status has gotten better. He is reacting very well to the antibiotics and the infections has decreased enourmously. He is not as short of breath but the coughing continues, although less strong. He underwent the endoscopy this morning and they found an obstruction in the esophagus, which is pretty big. They took a biopsy, which has been taken to the lab for analysis. At this point nothing is known about this obstruction. From the tomography done on tuesday, it was clear that the lung obstruction comes from this esophageal obstruction, but as they couldn't see outside the esophagus, Paul will have a brocheoscopy probably tomorrow, to see how much the lung access really has been compromised. The biopsy will tell us what this is and what we can do to reduce it. Surgical removal at this point is not an option.
I will keep you updated.

Lhes manterei informados.

Renée

First posting - English

Dear all,

Some of you already know that Paul has been hospitalized with breathing problems, but some of you I was unable to contact.

Paul has been complaining since May of a pain in the middle-left of his back. It progressed to a pain with a dry cough. It became harder to speak, as that triggered the coughing.

In June he was diagnosed with Pneumonia, although he never had a fever, and the new flu was also ruled out. He took 10 days of heavy antibiotics, which screwed up his stomach, but he did seem to improve. The dry cough however, remained.

He started to get short of breath with the coughing and started to show signs of asthma attacks. The doctor could not find anything wrong and gave him asthma medication, which seemed to help for awhile.

But, Paul ended up going to the emergency room 4x in the last 4 weeks because of this breathing difficulty! Two weeks ago they gave him a corticoid cure, which helped a few days. Then things got bad again.

Last monday he was at the ER again and apparently the pneumonia was back. Although the recommendation was to hospitalize, Paul reacted well to the intra-venous medication, so the doctor let him go home in the afternoon, with the promise of absolute bed-rest for a week, and strong antibiotics and cough medicines.

Things went well until wednesday. He didn't sleep too well that night and was more tired again. Although he was weak/tired on thursday and friday, when I left friday afternoon to teach, he was looking well, coughing less and generally on the mend. I spoke to Paul thoughout the weekend many times, and he kept insisting he was fine. When I got home sunday evening, he looked horrible. He had lost even more weight and could hardly speak due to the coughing. He had coughed up blood and started vomiting with the coughing spells.

Despite his resistance, we ended up in the emergency room again, and by 3am he was being hospitalized. He had gotten so weak he could not walk! He could not breath and had to get an internal nasal tube for oxygen, which he still is on. He was placed on an intra-venous glucose drip due to his dehidration and lack of strength.

The x-rays taken on Monday show that Paul's left lower lung has stopped working and his blood-work accused a huge bacterial infection. Both these things were not present the monday before. The infection is so strong that Paul will have to be on intra-venous antibiotics for 10 days. As these meds can not be done orally, he will have to remain hospitalized until the cure is done. The doctor has also ordered him to be on isolation. Not that he suspects the new flu, as that has mostly been ruled out specially because of the lack of fever and muscular pain, but because with this order he could be moved to a private room and I could stay with him.

Yesterday an imaging study was done, and they discovered that something is blocking the esophagus and the lower left lung. Tomorrow (thursday) they will be performing an endoscopy to see what it is and figure out what can be done to remove/reduce it.

I am at the hospital with him, most of the day. As soon as I have some more news, I will let you know.

Lots of love, and please keep Paul in your pray, positive thoughts and love.

Descartada novamente a suína.
